Offers transportation for follow-up medical visits.
On-site Mental Health Case Managers assess incoming residents and make appropriate referrals to mental health services. The Boulevard offers substance abuse assessment, one-to-one support and additional programming via their mental health partner, Mount Sinai Hospital, to serve clients who struggle with substance abuse.
The Boulevard partners with local HIV/AIDS outreach services to provide confidential on-site testing and counseling; access to primary care, psychosocial support programs, and medications; supportive housing referrals; and weekly HIV/AIDS/STD education programs.
